Sports Physicals
Students who are interested in playing a school sport next year must have a physical prior to tryouts. The physical must be dated on or after April 1, 2017 and on a high school league sports physical form. These forms can be obtained from the school nurse or any coach at CRMS. One physical can be used for all sports played at CRMS or Chesterfield High School for the upcoming school year.
Nurse Practitioners from Palmetto Family Medicine will be at CRMS on Thursday, Thursday, May 4th from 5 to 7 pm. Cost will be $20 (cash only). Physicals will be done by appointment only. If you would like for your child to receive a sports physical at school, please contact our school nurse for an appointment.
