Who Can Enroll:

Only the legal guardian of the student may complete the enrollment process.

Required Documents:

Please bring the following items with you to registration:

• Birth Certificate

• Certificate of Immunization (current and up-to-date)

• Proof of Residence (such as a utility bill — water, gas, or power)

• Social Security Card

• Proof of Custody (if applicable)

• Withdrawal Form from the previous school

Having all required documents at the time of registration helps ensure a timely and efficient enrollment for your student.
