CRMS will again host a clinic for students interested in obtaining a sports physical for the 2016–2017 school year. Students who are interested in playing a school sport next year and have maintained a passing average in all subjects this year are eligible to play, but must have a physical prior to tryouts. The physical must be dated on or after April 1, 2016 and on a high school league sports physical form. These forms can be obtained from the school nurse or any coach at CRMS. One physical can be used for all sports played at CRMS or Chesterfield High School for the 2016–2017 school year.
Nurse Practitioners from Palmetto Family Medicine will be performing the physicals at CRMS on Tuesday, May 17th from 5 to 7 pm. Cost will be $20 (cash only). Physicals will be done by appointment only. If you would like for your child to receive a sports physical at school, please contact our school nurse for an appointment.
