Who Can Enroll

Only the legal guardian of the student may complete the enrollment process.

Required Documents

Please bring the following items with you to registration:

• Birth Certificate

• Certificate of Immunization (current and up to date)

• Proof of Residence (such as a recent utility bill — water, gas, or power)

• Social Security Card

• Proof of Custody (if applicable)

• Withdrawal Form from the previous school
